Community-Based Services

PJBH believes all families have strengths, and each family member is integral to treatment. Our community-based outreach programs are designed to work with children, their families, and community members to reduce concerning behaviors while helping children, youth, and families meet their own unique goals and plans.

Service Overview:

A clinical evaluation was conducted to assess client needs, mental status, and diagnostic impressions. This process helps determine the appropriate services and treatments based on the presenting problem and any significant changes in the individual’s condition.


Who Can Participate:

Youth, adolescents, and adults aged 5 to 65 showing signs of mental illness.
 

The assessment results are shared with the individual to collaboratively develop a treatment plan and determine the necessity for further care. 


Based on your assessment, you will be placed in one of the following programs:

  • Inpatient Stabilization Program: For adults and geriatric populations.
  • Dual-Diagnosis Unit: Addresses psychiatric issues combined with drug and alcohol problems.
  • Outpatient Mental Health Day Treatment Program
  • Intensive Outpatient Program
  • Community Psychiatric Supportive Treatment (CPST) Service

Community Psychiatric Supportive Treatment Service

Our CPST staff works closely with individuals to ensure treatment needs are met, including
medication adherence, benefit acquisition, and accessing healthcare and housing resources.
This service bridges the gap between individuals and service providers, offering a
comprehensive, team-based approach to treatment. 


Who We Partner With:

Criminal justice systems, area hospitals, and social service agencies to provide a full range of
services.


Service Locations:

Services are provided in the community (homes, hospitals, jails, nursing homes) and at our
Cuyahoga County locations.

Therapeutic Behavioral Services and Psycho-social Rehabilitation

What is TBS?

A set of community-based clinical services aimed at improving social, communication, problem-solving, conflict resolution, and independent living skills.


Who Can Participate:

Youth, adolescents, and adults in need of mentoring and social skills development.


Model of Treatment:

The program utilizes cognitive-behavioral therapy (CBT) to help clients change their thoughts and behaviors. Social skills are essential for positive experiences at home, school, and in the community.


Program Structure:

Clients meet with therapeutic mentors in their homes, schools, or communities. The frequency of contact decreases as progress is made. The program’s duration varies based on individual needs, with regular evaluations to monitor progress.
